As the only dedicated community-based support service for stroke survivors- people with acquired brain injuries (ABIs) and their carers- loved ones & families- we aim to bridge the gap from when they leave clinical care- return home- face challenges of transitioning back into community & workplaces- challenges adapting to their new role as Carers- and seek to establish the “new norm”.
Peer social participation groups, skill development & assisted activities, social outings, recreational activities, community linkages & support to access the community, and community outreach. Through our Vocation Support Program, we help participants to establish vocation goals, increase their employability & confidently return to meaningful employment and/or further studies.
